Request a COI.
If your venue, planner, or municipality needs a Certificate of Insurance from My 3 Sons Fireworks Company before we can be confirmed for your event, we can issue one within two business days. Most COIs go out same-day during business hours.
What we need
- Certificate holder name (usually the venue or property)
- Certificate holder address
- Additional insureds (any entities that need to be named, typically the venue, property owner, planner, or municipality)
- Specific endorsements required (waiver of subrogation, primary & non-contributory, etc.)
- Event date and event description
- Coverage limits required if higher than our standard policy
How to request
Email the above to josh@3sonspyro.com with subject “COI Request, [Your Event Name]”. We’ll confirm receipt within one business day and send the certificate within two.
Existing booking?
If we already have your event on file, you only need to send the certificate holder details and any additional insured language. We’ll fill in the rest from your booking record.
